Participant Agreement
Waiver & Consent

I/We, the undersigned, affirm that I/We am/are the legal guardian(s) or parent(s) of the player enrolled.

Medical Authorization:
I/We grant permission to the staff of the WIMS Select Tryouts to obtain necessary medical care for the camper in the case of an accident, illness, or injury. This includes the right to administer first aid or contact emergency services if needed. I/We understand that any medical expenses incurred, will be the players family's responsibility.

Participation & Liability Release:
I/We confirm that our child is physically and mentally capable of participating in all soccer-related activities. I/We understand that participation in sports, including soccer, carries inherent risks such as physical contact, injuries, falls, weather-related challenges (even though training will take place indoors), and uneven field conditions. By signing this agreement, I/We voluntarily assume all such risks.

Furthermore, I/We hereby release and hold harmless Wolf Innovation Management Sports Inc., the WIMS Select Tryouts team, its staff, officers, representatives, and affiliates from any and all liability for injuries, damages, or losses that may occur during tryout activities or while on event premises, whether or not such incidents result from negligence.

I/We also acknowledge that refunds will not be issued in the event of injury.

Refund, Cancellation & Postponement Policy:
Refunds will not be provided. Instead, any registration fees paid will be held as a credit that may be applied to a future WIMS SELECT or other WIMS event.

Notification via Email/SMS: 

I/We grant permission to Wolf Innovation Management Sports inc. to send notifications via email/SMS about future events, tryouts or updates about WIMS SELECT. 

I/We grant permission to Wolf Innovation Management Sports inc. to give contact information to any sponsors of Wolf Innovation Management Sports inc.

Media Release for Promotional Use:
I/We authorize Wolf Innovation Management Sports inc. to capture and use photographs or video recordings of the player for promotional, marketing, or informational purposes, including but not limited to social media, websites, and printed materials.

Minor Media Consent:
I/We grant full permission for my/our child to be photographed or recorded during their participation in WIMS Select Tryouts for lawful purposes such as advertising, digital content, and promotional use. I/We understand that no compensation will be provided for the use of these materials.
